As a Community Health Worker at the Asian American Center of Frederick, you can help bring equity and access to community members who often face barriers. You will conduct outreach, prevention education, community engagement, standardized screening, care-barrier assessment, navigation, and care linkage across the Asian American Center of Frederick's community health programs. You will work across several grant-funded programs. Daily tasks could include: connecting families to pediatric care, WIC, Judy Centers, and FCPS Pre-K; and helping patients navigate cancer, diabetes, hypertension, and other chronic illnesses, including screening, prevention, and management. You will also play an integral role in connecting underserved populations with other appropriate community resources. • Run or co-lead community wellness circles.
• Conduct outreach and prevention education at community events, health fairs, faith-based settings, senior housing, libraries, and partner sites.
• Conduct standardized screening within the CHW scope.
• Complete care-barrier assessments and build individualized navigation and referral plans with priority needs and follow-up timelines.
• Make warm handoffs to primary care, behavioral health, social services, and — for family-focused programs — early-childhood and pediatric resources.
• Schedule appointments, send reminders, and follow up to confirm care connection and close referral loops.
• Track every contact and referral in our internal database and share information to partner databases as instructed.
• Step participants up to intensive coordination when needs intensify.
• Carry out program-specific duties as assigned across funded grants.

Our Ideal candidate* would have the following experience and skills:

• MDH CHW certification, or eligible to certify within the onboarding period. (AACF can support certification as an MDH-accredited CHW Training Center.)

• Comfortable with smartphones, tablets, and basic data entry.

• Reliable transportation for community-based work.

• Experience with health education and/or health promotion activities

• Strong communication and interpersonal skills

• Bilingual/multilingual abilities

• Experience working with immigrant and refugee populations and understanding of the unique challenges they face

• Strong knowledge of the healthcare industry 

• Knowledge of Frederick County resources and the Golden Mile / Frederick City communities. 

• Familiarity with local community resources and services for immigrants

• Excellent organizational and time-management skills

• Proficiency in Microsoft Office & Google Suite 

• Digital literacy, including the ability to use common computer applications and the propensity to learn and adopt new platforms quickly

• A valid driver's license

• A flexible schedule, some evenings and weekends will be required

• Community-based role with regular travel across Frederick City and County, including events and partner sites.

• Ability to lift light materials (e.g., outreach supplies) and work occasional evenings or weekends for events.

Job Type:

• Full-time; salaried 40+ hours per week 

• 50% office-based and 50% field-based, with evening/weekend work required

• Frederick, MD: Reliably commute or plan to relocate before starting work (Required)

Pay:

• $45,000 to $50,000 annually, depending on experience
